For Prospective University Scholars

Details on the components of the application are available on the Application Instructions page. The following forms should be used as part of the application:

• Learning and Project Plan (required of all applicants)
• Substitution Request Form (required of those applicants requesting general education or major course substitutions)

The University Scholar application is submitted via the Quest portal.

For Current University Scholars

• The Plan of Study Revision from (which also includes a section for any new substitution requests) should be submitted here for review and approval by your advisory committee. Be sure to discuss the changes and/or substitutions with your committee before submitting the form!
• The Final Plan of Study form should be submitted here for review and approval by your advisory committee.
• The Final Project should be submitted here for review and approval by your advisory committee.
